Faculty Qualification Review Documentation Template

This template is to be used by Department Deans to document instances where a faculty member is determined to be not qualified, or no longer qualified, to teach specific courses or within a particular discipline due to updated credential requirements or institutional policies. Complete all applicable fields below.

To use this template:

  1. Copy and paste the below template into a Word document.
  2. Input the applicable information on the specific faculty member.
  3. Save the document to your computer.
  4. Upload the document to the Faculty Qualification Form (FQV) in the Supporting Documentation section of the form.
    NOTE: Please submit a separate FQV form for the course(s) the faculty is not qualified for. One form is fine for all not qualified course(s), we just want to keep them separate from the course(s) the faculty may be qualified for.
  5. In the ‘Credentialed to Teach’ section of the form, please input the subject area(s) and course(s) that the faculty is not qualified for.
  6. The Dean and the AVP will check the ‘Does not meet minimum FQ’ box in the ‘Verification’ section of the form.

NOTE 1: Please create a separate qualifications form for not qualified courses. If you have multiple not qualified courses, they can go on one form, we just ask that they are not included on a form that has courses the faculty is qualified for.

NOTE 2: Once this routes through, the form will be uploaded into Perceptive and labeled NQF (Not Qualified) in the Year column of Perceptive.
